Myers Pharmacy Consultants HIPPA Policy

As a health care provider stated under the Health Insurance Portability and Accountability Act of 1996 section 186(u); Myers Pharmacy Consultants will adhere and follow the law to the fullest.

Myers Pharmacy Consultants will only communicate health information to authorized individuals.

Myers Pharmacy Consultants will only release health information when given a request in writing.

Myers Pharmacy Consultants will not fax or give health information via phone or oral communication to a third party without written authorization from the patient to whom the health information is pertaining to. The health information will only be release in writing.

Myers Pharmacy Consultants will provide the release authorization document for the release of protected patient information. To request an authorization document from please email mpcpa@myerspharmacyconsultants.com.

Myers Pharmacy Consultants will give protected patient information to third party payer to receive remuneration for service provided. Third party payer is an authorized health care insurance agent i.e. Humana, Blue Cross and Blue Shield, Cigna, Prudential, Aetna, AvMed, Well Care, stated or private insurer.

Myers Pharmacy Consultants will give protected patient information to law enforcement agency.

Myers Pharmacy Consultants will release protected health information in the case of circumstances that include reporting abuse of children, adults or disabled person. Investigations related to missing child also known as Amber alerts, internal investigations and audits by the department divisions bureaus and offices.
